HOW TO REGISTER WITH ABC

Please follow ALL the steps listed on this page

 

1. Please complete the intake form via the link below. Then, please read and sign the Policies and Procedures form.  

​

 

 

 

2. Upon submitting your online intake form, please pay your required fee via the links below.  **No cash or check accepted.** IMPORTANT: if the name on the card used for payment does not match your name, you MUST send us an email verifying agreement between both of you to process this payment. Failure to do so will halt your registration process until it can be verified.

3. Please email the following documents to info@accessbuildschildren.com:

 

Visiting (Non-custodial) Party

• An official photo identification (valid driver’s license, passport, etc.)

• Court order or mediated settlement agreement

• Picture of your child(ren) for our files

• A copy of any probation/parole/conditions of bond documents (if applicable)

• Proof of income (2 most recent paycheck stubs, 3 most recent bank statements, copy of previous year tax return or a letter from Texas Workforce)

  • If you are currently unemployed please download and complete the Zero Income Declaration form found at the bottom of this page.​

• Bank statements dated 3 months prior to intake date

Residential (Custodial) Party

• An official photo identification (valid driver’s license, passport, etc.)

• Court order or mediated settlement agreement

• Picture of your child(ren) for our files

• A copy of any probation/parole/conditions of bond documents (if applicable)

4. Email us at info@accessbuildschildren.com to schedule your intake meeting. Intakes will be conducted via Zoom and should last no longer than 30 minutes. Please email us with times and days you are available. Intakes will not be scheduled if the above steps are incomplete. ​

 

• If you are more than 15 minutes late to your scheduled appointment, it will be cancelled.​

• If you no show an appointment, cancel with less than 24 hours notice, or an appointment is cancelled due to arriving more than 15 minutes past your scheduled appointment time, then there will be a fee due prior to rescheduling.

5. After the intake, you will be required to take a quiz to ensure you fully understand the information presented to you. Visits will not be confirmed until both parties have passed their respective quizzes. It is imperative you pay attention during your intake. A link to the respective quizzes can be found below.
